Constellation backs Adaptive Innovations

Adaptive Innovations , an AI-native home health provider, has raised $60 million in funding, including a $50 million Series A round led by Felicis and Bain Capital Ventures and a previously raised $10 million seed round.

Other investors include Optum Ventures , Sunflower Capital, Conviction, BoxGroup, SV Angels, Dorm Room Fund, Constellation , and a group of healthcare services and AI industry angels.

Founded in 2025 by Alex Wendland, Logan Stinson, Ryan Tolsma, and Hunter Stinson, Adaptive combines AI-driven operational workflows with in-home clinical care. The company says its platform automates administrative functions such as intake, scheduling, documentation, billing, and compliance, helping reduce costs associated with home healthcare delivery.

According to Adaptive, administrative coordination costs account for between $0.60 and $0.90 for every dollar spent on clinical labor, contributing to an estimated $40 billion in rejected home health referrals annually.

Since launch, the company reports delivering more than 100,000 patient visits and partnering with over 500 referring healthcare organizations, including every major hospital system in Texas. Adaptive also claims to have reduced rehospitalization rates to 4.9%, compared to an industry average of 12.9%, while enabling clinicians to spend about 80% less time on documentation.

The company plans to use the funding to expand its team and enter additional states. Its workforce includes engineers and operators with experience at Scale AI, Palantir, Jane Street, and the U.S. Army Rangers, with operations based in New York City and Dallas.
